What To Bring
Please bring the following to your appointment:
• Your Medicare card and Private Health Insurance information
• A referral from your General Practitioner, family doctor or specialist
• A list of all medications that you are currently taking
• Details of any other treating doctors
We encourage new patients to come a few minutes earlier to fill out paperwork or you may download a Patient Registration Form and fill it out to bring with you.
If you need to cancel your appointment, please telephone the office during business hours and allow at least 1 days’ notice so that we can offer your appointment time to patients on our waiting list.
Confirmed appointments, cancelled at late notice, will incur a $120 cancellation fee. This amount is payable before another appointment can be made.
We recognise that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologise if we keep you waiting.
Please be aware that Dr Allen has a very tight schedule and arriving late for your appointment can inconvenience subsequent patients. Should you arrive very late for your appointment, it may need to be rescheduled as Dr Allen would not be able to give you the time that you need.
We can submit your claim directly to Medicare using our online claiming facility. If you are registered with Medicare and have recorded your bank details with them, your Medicare rebate can be directly deposited in to your nominated bank account if requested. This facility is subject to availability as sometimes the Medicare systems are down.
Medicare Australia rebates approximately 45% of the fee for our services (Based on 85% of the scheduled fee). Once you reach the Medicare safety net threshold you may be eligible for additional Medicare benefits for the remainder of the calendar year. To find out more about the safety net click here
From July 1, 2016 Medicare will no longer issue refund cheques. Your refund will only be available to be deposited into your nominated bank account. If you have not notified Medicare of your nominated bank account number, you will not receive your refund.
If you have a pre-arrangement with our Doctor, phoned prescription requests incur a $16.00 administration fee payable at the time of request. This is payable over the phone via credit card. We can fax and post this directly to your pharmacy if you prefer. Please have your pharmacy details available for our staff.
Dr Allen will not provide any information about you to any third parties without discussing the matter with you first. This includes requests from insurance companies or solicitors even if they have a signed consent from you.
If you wish Dr Allen to speak to your partner or relatives, Dr Allen will need to discuss this with you first. Dr Allen will not accept telephone calls or emails from any relatives or partners.
If a court issues a legal subpoena then Dr Allen is bound to release documents to them. This sometimes happens in family law proceedings. Please see also our Privacy Page for further information.
Dr Allen is happy to provide information or a summary about your care to another doctor as long as you consent to this in writing. There may be a fee incurred, depending on the complexity of the information. We do not provide copies of the complete chart.
